Claims

Sign up for Tapestry Link (TapLink) to easily submit claims. Our Tapestry Link portal can be used to submit claims as well as view claim status and eligibility* for your Quest members. Claims entered through Tapestry Link can reduce turnaround time on clean claims to just a few days. If you would like more information or wish to proceed with using Tapestry Link, please complete the Site Access Request form linked to the right. *Benefit details relate only to the individual member’s coverage and are not provider specific. Quest recommends continuing to call 800-364-6352 to verify benefit information. Alternative Options for Claim Submission Quest requests all providers submit claims electronically. Paper claims will be accepted by exception only to the appropriate PO box below.

Payments

Receive secure electronic payments through our partnership with Zelis. We offer electronic payment and remittance advice options through Zelis. This alliance provides the latest in secure ePayment technology to accelerate and add efficiency to our claim payment process.

If you are interested in exploring options for ACH payments and electronic remittance, please use the link to the right to enroll.

For additional information or assistance with registration, please contact Zelis Payments’ Client Service Department at 877-828-8770.

Paper checks and/or an Explanation of Payment (EOP) will be provided for all claims processed if an electronic payment option is not selected.

Member Benefit Request Form

What if I have questions regarding a claim?

You may submit questions regarding claim status online with our Claim Status Inquiry form, by sending a secure email to our Claims Center at claims@questbh.com, or by calling Quest at 800-364-6352.

What if Quest is the secondary insurance?

Quest requires an EOB (Explanation of Benefits) from the primary insurance to process claims.

How can I determine the status of my claim submission?

To determine the status of your claim, complete the Claim Status Inquiry Form or call the Quest Claims Department at 800-364-6352 or 717-851-1480.

I'm having issues with your online portal, Tapestry Link.

How do I submit claims?

Does Quest reimburse for incident-to billing?

(Incident-to billing is a Medicare billing mechanism that allows supervising physicians to bill for services that other healthcare team members provide in office or home settings). Quest does not reimburse for incident-to billing.

How do I determine a member’s benefits?

Providers may request membership eligibility and benefits online using our Member Benefit Request form or contact us at 800-364-6352.

I received an email from appcentral@cactussoftware.com related to credentialing; is this valid?

If you received an email from the address with a subject of “Reappointment application with Quest is ready”, this is a valid email requiring your response.

How will I receive my 1099 tax form?

1099 forms are issued from our payment vendor Zelis.

What are the requirements for submitting claims for autism services?

Please contact us at 800-364-6352 for further assistance, or you can email our team at claims@questbh.com.

Can I bill for my services if I don’t have a valid license?

Quest does not reimburse unlicensed providers.

What are the requirements for providing services for a member residing in another state?

Providers must be licensed in the state in which the member is located at the time of service.

How do I submit a corrected professional claim?

On a CMS-1500 claim form, enter resubmission code 7 and the original claim number in box 22. Note that resubmitted claims should include all original content submitted with corrections made where applicable.

How do I submit a corrected facility claim?

On a UB-04 claim form, enter 7 as the third digit in field 4, type of bill. Note that resubmitted claims should include all original content submitted with corrections made where applicable.

How can a member update their coordination of benefit information?

Please have the member contact us at 800-364-6352 for further assistance.

How can I request an explanation of payment (EOP)?

Electronic EOPs and payment options are available through Zelis.
